Background check authorization

Optional. Signing now lets staff move faster if a background check is needed for a specific unit — you can skip it and MACV will follow up with you later.

Minnesota Assistance Council for Veterans (MACV), or the property where you are applying to live, may obtain a consumer report about you for the purpose of this housing application. That report may include rental history, credit history, and criminal record information, and is prepared by a consumer reporting agency (Rental History Reports).

Something showing up on a report does not automatically disqualify you. MACV housing exists to serve veterans coming from homelessness, and staff will talk with you about anything that comes up and work with you on next steps.

You have the right to request a copy of any report that is obtained about you, and to dispute information you believe is inaccurate or incomplete with the consumer reporting agency that furnished it.

You may submit this application without signing this authorization. If you do not sign, MACV staff may follow up with you later if a background check is needed for a specific unit.
